I used to take little turbo-props in and out of Mammoth, runway elevation 7,129 ft. Lucky the longest flights were less than 300 miles, otherwise they couldn't carry enough fuel to get there. It was the only place I've ever been weighed before getting on a plane.
Flying in and out of Telluride can be a bit of an adventure- between the altitude, the short runway (that drops over a cliff) and the weather that turns on a dime, it can be real tricky getting in and out of there. I've also had problems at the Gunnison airport where the altitude and the cold can make for a fun takeoff- we once had to circle back three times for deicing because it kept freezing over before we could take off
